Our client has a locum job vacancy for a Community Care Assessor to work within a dementia and review team based in South London. This vacancy has an initial 3 month contract and the pay rate for this role is £16 per hour (limited company worker).
Duties include:
- Undertaking community care assessments, case work, support plans, reviews and discharge planning
- Assisting vulnerable adults and their families/carers
- Assessing and meeting individuals' needs
- Identifying risks
Requirements:
To be eligible for this role, you must have a degree within social work and be registered with the HCPC. You must also have at least 2 years recent post qualifying experience working within a similar role.
