Social Worker - Hospital Discharge Our client has a job vacancy for a Social Worker to work within a Hospital Discharge Team based in Burton on Trent. This vacancy has an initial 3 month contract and the pay rate for this role is £21.96 per hour (limited company worker). Requirements: Have you got a social work degree or equivalent? Are you HCPC registered? Do you have at least 2 years recent post qualifying experience working within Hospital Discharge? Duties: - You will be facilitating discharge, arranging packages of care, temporary placements. - You will be participating in staff meetings when necessary, attending and participating in care/case reviews/conference as required. - You will be undertaking assessments and reviews of cases.
