Band 6 Care Coordinator - Adults Community Mental Health Our client has a job vacancy for a Band 6 Care Coordinator to work within an Adults Community Mental Health Team based in North Cumbria. This vacancy has an initial 3 month contract and the pay rate for this role is £25-30 per hour depending on experience (limited company worker). Duties: - Assessing and coordinating care need. - Planning and implementing evidence based care. - Working with a defined group of service users. - Managing a client caseload. - Promoting positive mental health. - Providing safe, sound and supporting mental health services. - Maintaining high standards of individualised care. Requirement: - All applicants must have a Registered Mental Health Nurse first level registration, valid NMC registration; - Or, Dip SW or equivalent qualification for other professions. - A minimum of 1 years post qualifying experience working within a similar role.
