Social Worker - Community Mental Health Our client has a job vacancy for a Social Worker to work within a Community Mental Health Team based in Powys. This vacancy has an initial 3 month contract and part time will be considered. The pay rate for this role is £20-23 per hour (limited company worker). Requirements: Have you got a social work degree or equivalent? Are you registered with the CCW? Do you have at least 2 years recent post qualified experience working within a Community Mental Health Team? If you fit these requirements then you could be the ideal candidate. Duties: - You will be assessing the needs of service users. - You will be attending CPA reviews. - You will be working with multidisciplinary team members. - You will be attending weekly referral meetings. - You will be coordinating multidisciplinary meetings. - You will be assessing, planning, implementing and evaluating care plans.
