Position: Community Liaison Nurse

Location: London

Salary: £26,000 - £32,000 fuel card



I am currently working with a company that specialises in providing complex care to patients within their home environments. These patients suffer from conditions such as spinal or brain injury and require constant 24 / 7 care.



My client is looking for a Community Liaison Nurse to work within London. Your responsibilities will include:



* Managing and supporting Support Workers and training them in specialist areas such as skin care and bowel management

* Writing care plans and tailoring them to individuals

* Liaising with community mainstream services

* Supporting care managers and working with families

* Hands on nursing
