Social Worker Adults Hospital Team



Medacs Healthcare is seeking applications on behalf of their Client, a Local Authority, for a Qualified Social Worker to work within a Hospital Team. Candidates will require a minimum of 2 years post qualified experience of working in a Qualified Social Work role within adult's services. Duties include: Attend MDT meetings with OT's / Physios and assist with providing assessment and care management to range of individuals being discharged from hospital Conduct follow up visits for a 2 week period after discharge, to check on patient Desirable Qualities: Experience of working in an acute hospital setting Minimum requirements: Social Work Diploma or Degree HCPC registration Car driver with access to a vehicle Minimum of 2 years experience to include solid knowledge of adult Safeguarding. Medacs Healthcare operates a 'refer a friend' scheme, and so if you know of someone else who may be interested in registering with us, you will receive £200 referral fee once the recommended person has been successfully appointed into a role for a minimum 12 week assignment. This is a 3 month temporary contract. If you do not receive a reply within 5 working days, then your application has not met our Client's specified criteria and will not be progressed further on this occasion.
